how to make fidgets?

i made a post about how i can't really stop picking my skin and bleeding and the suggestions were to get fidgets,specifically pain/spiky ones.

and i don't wanna buy them so does anyone know how to make one of those spiky roller thingys?i was thinking maybe beads or like the ends of toothpicks glued to a tube,but i'm not sure if its supposed to be sharp or just kinda pointy?

also does anyone wanna reccomend specific ones? no picky pads though cuz those make me super uncomfortable with the holes the beads and whatnot leave behind in the silicone.
